Office location:

My office is in Suite 208 of the former American Textile Museum located at 491 Dutton Street in Lowell, MA 01854.  

Enter the glass front doors of the building and proceed across the lobby to the Kitson Business Center.  Enter and take a hard left to the elevator.  Take it to the second floor, exit to the left, and you will see my office around the corner.  When you arrive for your appointment, please enter the suite door, have a seat in the waiting area, and I will greet you at your appointment time.

 

Parking:

There is free, one-hour parking in the lot outside the building for client use.

 

For your first appointment:

I advise all new clients to call the member service phone number on the back of your insurance card prior to your first appointment to ask, "What will my out-of-pocket costs be?"  

Please bring your insurance card to your first appointment if you would like me to bill your insurance company.  Depending on the details of your insurance plan, you may have some out-of-pocket costs such as a deductible or a copay.  If you have an unmet deductible, payment for the full session cost is due at each visit until your deductible is satisfied.  Copays are due at the time of your appointment. 

Cash or check is preferred.  I also accept flex-spending accounts, Visa, and MasterCard if needed.

 

Ongoing:

Please let me know as soon as possible if you have any changes to your insurance policy while we work together.  You could be responsible for charges out-of-pocket if I am not in-network for your new insurance, if your new policy has a deductible, or if your copay has changed, to illustrate a few examples.   Please communicate insurance changes right away to avoid surprises.
